The Influence of Physical Conditions of Homes, Prevention Behavior, and Compliance with Drugs on Transmission of Lung TB Diseases in the City of Banda Aceh in 2015

Introduction

Lung Tuberculosis (TB) is one of the most serious public health problems worldwide. In the city of Banda Aceh, despite the government's efforts to provide free treatment for pulmonary TB sufferers, the number of new cases each year remains high, indicating that the prevention program is not yet fully effective. This study aims to analyze the effect of home sanitation, preventive behavior, and compliance in taking drugs on the transmission of pulmonary TB disease in 2015.

Background

Pulmonary TB is a major public health concern in many parts of the world, including Indonesia. According to the World Health Organization (WHO), TB is one of the top 10 causes of death worldwide, and Indonesia is one of the countries with the highest TB burden. In Banda Aceh City, the number of new TB cases each year is significant, and the disease is often transmitted through close contact with infected individuals.

Research Methods

This study uses an analytical survey method with a control case design. The number of samples taken consisted of 31 respondents diagnosed with pulmonary TB and 62 respondents as a control group. Data were analyzed using the Chi-Square test for bivariate analysis and logistics regression analysis for multivariate analysis.

Research Results

The results showed that there were several variables that had a significant effect (p <0.05) on the incidence of pulmonary TB transmission. These variables include:

Knowledge: Respondents who had knowledge about TB and its transmission were less likely to contract the disease.
Attitudes: Respondents who had positive attitudes towards TB prevention and treatment were less likely to contract the disease.
Preventive measures: Respondents who practiced preventive measures, such as wearing masks and avoiding close contact with infected individuals, were less likely to contract the disease.
Residential density: Respondents who lived in densely populated areas were more likely to contract the disease.
Ventilation: Respondents who lived in homes with poor ventilation were more likely to contract the disease.
Lighting: Respondents who lived in homes with poor lighting were more likely to contract the disease.
Type of floor of the house: Respondents who lived in homes with wooden floors were more likely to contract the disease.

Analysis

The physical condition of the house, including residential density and sanitation, has an important role in spreading pulmonary TB disease. A narrow house and does not have good ventilation tends to increase the risk of transmission. This shows the need for more attention to decent residential arrangements and health-friendly for the community. In this context, good prevention behavior, including compliance in consuming drugs, is also a key factor in breaking the chain of transmission.

Recommendations for the Health Office

The Health Office is expected to be able to establish cross-sector cooperation with related institutions, such as the Social and Housing Service. This is to ensure that people get access to decent residences. In addition, the Puskesmas needs to hold counseling and training to increase the knowledge and attitudes of the community, especially for those who are in high-risk groups. Health workers also play an important role in conducting routine visits to residents' homes to find new cases, and encourage people to be more active in following routine treatment. Health promotion media can be used to provide broader appeals so that people understand more about the importance of maintaining health and preventing transmission of pulmonary TB.

Conclusion

Handling of pulmonary TB in Banda Aceh City requires a holistic and integrated approach, involving the community, the Health Office, and other related institutions. By improving the physical condition of the house, preventive behavior, and compliance with drugs, it is expected to reduce the number of pulmonary TB transmission in this area.

Limitations of the Study

This study has several limitations. Firstly, the sample size was relatively small, which may limit the generalizability of the findings. Secondly, the study only focused on pulmonary TB, and did not consider other types of TB. Finally, the study only analyzed the effect of physical conditions of homes, prevention behavior, and compliance with drugs on the transmission of pulmonary TB disease, and did not consider other factors that may contribute to the transmission of the disease.

Future Research Directions

Future research should aim to build on the findings of this study by exploring other factors that contribute to the transmission of pulmonary TB disease. For example, research could investigate the role of socioeconomic factors, such as income and education level, in the transmission of the disease. Additionally, research could explore the effectiveness of different interventions, such as health education and promotion, in reducing the transmission of pulmonary TB disease.
