Introduction Cardiac rehabilitation is a crucial component in the management of patients after undergoing coronary artery shortcut surgery (BPAK). The primary goal of cardiac rehabilitation is to help patients recover from heart disease and reduce the risk of future cardiovascular events. One important indicator of the response to exercise is the heart rate recovery (HRR). HRR is a simple method and is able to provide significant information about increased risk of death and rehospitalization due to cardiovascular disease. This study aims to explore the relationship between recovery of heart rates with the risk of death and rehospitalization in BPAK patients who have undergone a phase II cardiac rehabilitation program.

The Importance of Heart Rate Recovery Heart rate recovery is a simple and non-invasive method that measures the time it takes for the heart rate to return to normal after exercise. It is a reliable indicator of cardiovascular function and can provide significant information about the risk of death and rehospitalization due to cardiovascular disease. Studies have shown that abnormal heart rate recovery is associated with an increased risk of cardiovascular events, including heart failure, myocardial infarction, and stroke.

Methodology This study uses a retrospective cohort design involving 104 BPAK patients who have participated in the Heart Rehabilitation Program Phase I and II from January 2019 to February 2020. Heart rate recovery is defined as the difference between the maximum heart rate and heart rate during rest in the first minute after exercise. Criteria for abnormalities of recovery of the heart rate is determined based on a certain threshold value. The component studied is a combination of death and rehospitalization, and the patient is followed for 3 months after the Phase II cardiac rehabilitation program is complete.

Results Of the 104 patients studied, there were 18 patients (17.3%) who experienced a combination of death and rehospitalization, with heart failure as the most common cause (38.2%). The analysis shows a significant relationship between the value of heart rate recovery and the combined event of death and rehospitalization (p <0.001). Multivariate logistics regression test shows that abnormalities in the recovery value of the heart rate have statistically significant relationships to the combination of death and rehospitalization (Odds Ratio: 35.42; 95% Intervals of Trust: 4.8-261.25; P <0.001).

Conclusion Post-rehabilitation heart rate recovery phase II can function as an indicator of predictors in estimating the combined events of death and rehospitalization in BPAK patients. This discovery can help in evaluating the risk and post-surgical management in patients with heart disease, as well as contributing to the development of more effective and focused rehabilitation programs to improve patient health outcomes.

Implications for Practice Through this study, it is expected that health professionals can better understand the importance of recovery of heart rates as an evaluation component in cardiac rehabilitation and long-term risk management in post-BPAK patients. As a further action, it is important for hospitals and health institutions to integrate HRR measurements in rehabilitation programs to improve the safety and quality of life of cardiovascular patients.

Limitations This study has several limitations, including the small sample size and the retrospective design. Future studies should aim to recruit a larger sample size and use a prospective design to confirm the findings of this study.
