Introduction

Diarrhea is a global health problem that affects millions of people worldwide, especially in developing countries like Indonesia. As a major public health issue, diarrhea requires effective treatment to prevent complications and improve patient outcomes. However, the treatment of diarrhea in pediatric patients can be challenging due to the complexity of drug interactions and the need for precise dosing. Drug-related problems (DRPS), also known as adverse drug events, are undesirable events experienced by patients related to drug therapy. These events can affect the results of treatment and patient safety. This article aims to identify and analyze DRPS in the treatment of diarrhea for pediatric patients in the inpatient installation of Dr. Pirngadi Medan for the period January-June 2017.

Background

Diarrhea is a common gastrointestinal disorder that affects people of all ages, but it is more prevalent in children. In Indonesia, diarrhea is one of the main public health problems, and it is estimated that over 1 million children under the age of 5 die from diarrhea-related illnesses each year. The treatment of diarrhea requires careful consideration of the patient's age, weight, and medical history to ensure effective and safe treatment. However, the complexity of drug interactions and the need for precise dosing can lead to DRPS, which can have serious consequences for patients.

Research Methods

This study is a descriptive research that uses retrospective data to analyze DRPS in the treatment of diarrhea for pediatric patients in the inpatient installation of Dr. Pirngadi Medan. The data were collected from medical records of patients who were treated for diarrhea between January and June 2017. A total of 70 medical records were examined, and 49 of them met the inclusion criteria. The data were analyzed using the DRPS classification according to the modified pharmaceutical maintenance practice guidelines.

Research Result

The results of this study show that there were 32 cases of DRPS in the treatment of diarrhea for pediatric patients in the inpatient installation of Dr. Pirngadi Medan. The types of identified DRPS are as follows:

Drug Interaction: 14 cases (31.81%)
Drug dosage is too high: 8 cases (18.18%)
Medicine without indications: 8 cases (18.18%)
Indications without drugs: 8 cases (18.18%)
Drug dose is too low: 6 cases (13.63%)

Analysis

The findings of this study show that DRPS are significant issues in the treatment of diarrhea for pediatric patients in Dr. Pirngadi Medan. Drug interactions are the most common problems encountered, which can potentially cause serious side effects or reduce the effectiveness of therapy. In the treatment of diarrhea, the right choice of drugs is very important to achieve the desired results and minimize the risk of side effects.

In addition, the case of drug doses that are too high or too low indicate the need for more attention from health workers in prescribing drugs for pediatric patients. Inappropriate doses can cause negative impacts on children's health, such as drug poisoning or lack of therapeutic response.

Conclusion

Based on the results of this study, it can be concluded that there has been a drug-related problem (DRPS) in the treatment of diarrhea in pediatric patients in the inpatient installation of Dr. Pirngadi Medan during the period January to June 2017. Better handling of DRPS is needed to improve the quality of treatment and patient safety. Efforts to educate health workers about the importance of DRPS identification and management are very necessary so that treatment can be more effective and safe.

With a better understanding of DRPS, health workers are expected to be able to conduct appropriate interventions in diarrhea treatment, especially in vulnerable populations such as children, to improve overall health results. This study highlights the importance of DRPS identification and management in the treatment of diarrhea for pediatric patients and provides a basis for further research in this area.
