Introduction

The Clean and Healthy Behavior Program (PHBS) is a crucial initiative aimed at improving public health quality. In South Tapanuli Regency, the KKG (Mutual Assistance Working Group) project implemented the PHBS program in 2004. The evaluation results showed that the PHBS level of the household order in this region was divided into three classifications: 22.4% reached classification IV, 65.2% achieved classification III, and 12.4% reached classification II. This study aimed to identify the factors that affect the achievements of the PHBS level of the household in the KKG project site in South Tapanuli Regency.

Methodology

This study employed a cross-sectional design involving 138 respondents taken through quota sampling in three sub-districts at the KKG project site. Data collection was carried out through questionnaires, and multi-logistic test analysis was used to test the relationship between research variables.

Results

The results showed that the variable age, knowledge, action, and directed group discussion (DKT) significantly affect the level of PHBS Classification IV. There is a significant interaction between the action variables and age after being controlled by the DKT variable and knowledge. Respondents aged ≤ 40 years, with good knowledge, taking good actions, and following the DKT have a probability of an increase in the level of PHBS household order to Classification IV by 55.9%.

Discussion

This finding shows that efforts to increase the classification of PHBS The household order to Classification IV must involve strategies that focus on young age groups (≤ 40 years) and increase their participation in DKT. Increasing knowledge and awareness about PHBS among young people is very important, as good knowledge will encourage proper actions and have a positive impact on the level of PHBS household order.

Strategies that can be applied by the South Tapanuli District Health Office to increase the classification of PHBS household order include:

Strengthening the Role of Health Cadres

Strengthening the role of health cadres in providing education and assistance to the community. Health cadres can become effective agents of change in encouraging the application of PHBS at the household level.

Conclusion

With a directed and comprehensive effort, it is hoped that the classification of families at the South Tapanuli Regency KKG project will increase to Classification IV, so that the status of PHBS regional level can be increased from a healthy area 1 to a healthy area 2 and so on.

Limitations

This study has some limitations. The sample size was relatively small, and the study was conducted in a specific region. Future studies should aim to replicate this study in other regions and with larger sample sizes.

Future Directions

Future studies should focus on identifying the factors that affect the PHBS level of the household in other regions and developing strategies to improve the classification of PHBS household order. Additionally, studies should investigate the effectiveness of the strategies recommended in this study.
